Terms of Service

1. Introduction and Acceptance of Terms

Welcome to Precision Metabolic Health PLLC ("we," "us," or "our"). These Terms of Service ("Terms") govern your use of our website and telehealth services. By accessing our site or booking an appointment, you agree to follow these rules. If you do not agree, please do not use our services. These Terms are a legal agreement between you and Precision Metabolic Health PLLC.

2. Who We Are and Eligibility

Precision Metabolic Health PLLC is a Colorado-based telehealth clinic specializing in metabolic and hormone health. Services are exclusively available to residents of Colorado, USA. You must be at least 18 years old to use our services independently. Minors may be seen only with the direct presence and written consent of a parent or legal guardian, and at the clinical discretion of our providers.

3. Telehealth Nature and Technology Limits

We provide medical care entirely through electronic communications, including video visits, secure messaging, and our patient portal. While telehealth is convenient, it has limits. Technology failures, internet interruptions, or security breaches (though we use industry-standard encryption) can occur. By using our services, you acknowledge that remote care is not the same as an in-person exam and carries inherent technical risks.

4. Not Emergency Care

OUR SERVICES ARE NOT FOR EMERGENCIES. If you are experiencing a medical or mental health emergency, call 911 immediately or go to the nearest emergency room. We do not provide 24/7 crisis monitoring and cannot respond to urgent medical needs outside of scheduled business hours.

5. Medical Information Disclaimer

The content on our website is for informational purposes and is not a substitute for professional medical advice or emergency care. While we strive for clinical excellence, we provide no guarantees of specific health outcomes or results. Medical decisions are complex, and individual responses to therapy vary.

6. Your Responsibilities

  • As a patient, you are responsible for:
  • Providing accurate and complete health information.
  • Following the clinical instructions provided by your provider.
  • Using the Charm HIPAA-compliant portal exclusively for all health-related communications and document sharing.
  • Keeping your portal login credentials secure and private.
  • Ensuring timely payment for all services rendered.

7. Payments, Fees, and Refunds

Our practice operates on a pay-as-you-go model. Payment is due at the time of service. We do not bill insurance directly. You are responsible for the full cost of your care. While you may request a superbill to submit to your insurance for potential out-of-network reimbursement, we provide no guarantee that any service or medication will be covered or reimbursed by your insurance provider.

8. Scheduling, Cancellations, and No-Shows

Appointments must be scheduled through our portal or website. If you need to cancel or Reschedule, you must do so at least 24 hours in advance. Cancellations made with less than 24 hours' notice, or "no-shows," may be subject to a cancellation fee as outlined in our current fee schedule.

9. Prescriptions and Pharmacy Use

Prescriptions are issued based on clinical necessity. We may utilize compounded medications from specialized pharmacies. Be aware that some compounded medications may not be FDA-approved. Your provider will explain any specific risks or benefits of these treatments during your care. You maintain the right to choose your preferred pharmacy for standard medications.

10. Privacy and Security

Protecting your health information is our priority. We comply with HIPAA regulations and use secure technology to manage your records. For a complete understanding of how we handle your data, please review our full Privacy Policy [link text to Privacy Policy page].

11. Limitation of Liability and No Guarantees

To the fullest extent permitted by law, Precision Metabolic Health PLLC and its providers are not liable for any indirect, incidental, or consequential damages arising from your use of our services. We do not guarantee specific results, and all clinical care is provided on an "as-is" basis centered on established medical standards.

12. Governing Law and Location

These Terms are governed by the laws of the State of Colorado, USA. Any disputes arising from these Terms or our services will be resolved exclusively in the courts located in Colorado.
